Job Description Summary:The SpeechLanguage Pathologist is responsible for implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement.Essential Job Functions/Responsibilities:A.Conducts appropriate evaluations, assesses home environment and identifies equipment needs relative to speech/language function.B.Identifies functional speech defects and establishes a plan of care to improve patients function.C.Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.D.Recommends communication devices/aids as indicated.E.Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.F.Participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.G.Prepares and submits notes per organization policy.H.Maintains clinical competency in speech language pathology practice and theory.I.Identifies patient and family/caregiver needs for other services and refers as appropriate.J.Instructs patient, family/caregiver and other Organization health care personnel in the patients treatment regime as indicated.K.May supervise home health aide in procedures directly related to the speechlanguage therapy plan of care.

Job Requirements

1.Possesses a degree that includes an internship from an approved Masters program in SpeechLanguage Pathology, or as required by applicable law and/or regulation.2.Meets the education and experience requirements for a Certificate of Clinical Competence in SpeechLanguage Pathology, granted by the American SpeechLanguageHearing Association (ASHA).3.Is certified to practice SpeechLanguage Pathology.4.A minimum of one years clinical experience in SpeechLanguage Pathology.5.Demonstrates good verbal and written communication, and organization skills.6.Possesses and maintains current CPR certification.Continuing Education Requirements: Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs.
