Social Worker (LMSW)/Bereavement Coordinator

BeaconHospicehas an opening for aSocial Worker(Must be LMSW).

Our Social Workers:

• Provide routineto patients and or family or caregiver(s) in private residences, skilled nursing facilities, assisted living facilities, residential/group homes, and/or hospitals
• Provide ongoing assessments (to include preparatory bereavement assessments), support, active listening, problem solving, conflict resolution, crisis management, individual and family counseling,financial counseling, education and assistance with Advance Directive Planning, education and assistance with DNR status, assistance with funeral arrangements, assistance with community referrals and a minimum of monthly caregiver phone calls.

TheSocial Workeralso works with patients and family in all aspects of the grieving process and funeral arrangements as well as to recruit, manage and train volunteers from the community.

Job Requirements

• Bachelors degree in Social Work
• Masters level degree in Social Work, Psychology, Sociology, or Counseling preferred
• Must be licensed in Social Work
• At least one year healthcare experience
• Experience with grief issues
• Experience working with support groups
• Experience working with the community and various outreach programs
• Demonstrates self-direction, flexibility and organizes workload efficiently.
• Continually display a positive attitude and professional manner/demeanor.
• Works well in a team environment
• Reliable transportation and a clean driving record.
• Proof of current car insurance coverage.
