We are looking for a dedicated Hospice Social Worker to work as part of our motivated healthcare team.
Job Responsibilities
As a Hospice Social Worker you will be an integral part of the hospice interdisciplinary care team and responsible for advocating for clients and patients to resolve crises.
Additional responsibilities of the Hospice Social Worker include:
- Participating in the development of the plan of care for each hospice patient and provides direct social work services
- Assessing the patient/family psychosocial response to terminal illness
- Participating in quality improvement programs
- Assisting patient/family and agency staff in utilizing community resources
- Communicates with team members to provide and gather information to develop the family bereavement care plan and to coordinate needed services
- Exploring and evaluating financial situation and identifying appropriate resources as needed, documents all interventions accurately, completely, and on a timely basis
- Facilitates bereavement group activities
- Assisting team members in understanding the significant social and emotional factors related to care of a terminally ill patient and family

To qualify for the Hospice Social Worker you must have previous experience working with adult clients in a hospice healthcare environment.
Additional requirements of the Hospice Social Worker include:
- Current and valid Social Work (LMSW) license in state of requested employment
- 3 years’ experience in hospice or home health care environment

Meets Federal and State requirements for providing bereavement services
- Current and valid driver’s license
